Some plans need one from your child's doctor. We'll tell you if yours does when we check your benefits.
Yes, if we serve your area and your plan allows it. With your OK, we'll ask your current provider for records so your child doesn't start from scratch.
It depends on your plan's approval and on staffing in your area. We'll keep you posted, and we won't promise a date we can't keep.
